Why ACL rehab needs strength training

An ACL reconstruction is one of the most demanding rehab journeys you can go through. The quadriceps can lose 20 to 30% of their cross-sectional area in the weeks after surgery. Even with excellent physiotherapy, that muscle doesn’t fully come back without structured loading.

The athletes who return to sport and stay back tend to share one thing: they did full strength rehab, not just early-stage physio. The athletes who suffer re-injury or end up with a “weak knee” for years are usually the ones who stopped at stage 2.

The Strength Lab LDN ACL rehab programme is built to bridge that gap – taking over from your physio once you’re cleared for loaded exercise, and rebuilding the knee to full capacity.

The four stages of ACL rehab

Stage 1: Early recovery (weeks 0-6)

Physio-led. Swelling control, range of motion, gentle isometric activation, protected weight bearing. No loaded strength work yet.

Stage 2: Foundation (weeks 6-12)

S&C work begins. Bodyweight squats, glute bridges, step-ups, light loading, gradual tissue tolerance building. This is where most Strength Lab clients start with us, working alongside their physio.

Stage 3: Strength (months 3-6)

Loaded compound movements – back squat, front squat, deadlift variations, single leg work, progressive overload. This stage is critical. Skipping it is the most common cause of long-term weakness.

Stage 4: Performance / return to sport (months 6-12)

Power, plyometrics, sprint mechanics, change of direction work. Return to sport testing. Full restoration of athletic capacity.

What’s included in the £140 programme

1. Initial S&C assessment

Full assessment of movement, strength, history, and goals. We look at how the operated knee moves and loads compared to the other side, where the weak points are, and where you are in your timeline.

2. One-month tailored programme

A bespoke training programme based on your assessment. Progressive loading, sport-relevant where appropriate, adapted weekly based on how the knee is responding.

3. Final consultation and assessment

Repeat assessment at the end of the month. We measure progress objectively and plan the next phase – whether that’s continuing rehab, transitioning into personal training, or moving toward return to sport.

Pre-surgery ACL rehab (prehab)

If your ACL surgery is still weeks or months away, prehab is one of the best uses of that time. The evidence is consistent: the stronger your quadriceps, hamstrings, and hip stabilisers going into surgery, the faster and more complete your recovery typically is.

We work with clients in the weeks and months before scheduled ACL surgery, building strength, technique, and conditioning that pays off after the operation.

Physio or S&C – which do I need?

Both, at different stages.

A physiotherapist manages the early medical stages: post-op care, swelling, range of motion, gait re-training, and clearance for loaded exercise. They are the right person for weeks 0-6 minimum, and often longer.

A strength and conditioning coach takes over from stage 2 or 3 onwards. S&C is a different discipline – focused on building strength, power, and movement capacity rather than treating tissue. Strength Lab LDN coaches are not physios.

The best outcomes happen when both work together. We’re happy to coordinate with your physio and share programmes if helpful.
